Social Service Coordinator- Part-time

Schedule: Part-time, Monday through Friday, Flexible

Reports To: CEO

Position Summary:
Responsible for consultation and direction of social services in the acute and skilled nursing facility. Provides orientation and education to the social service and nursing staff. Develops, organizes, and implements programs, policies and procedures, conducts case work services and counseling and recommends functions necessary to ensure overall operational viability. Maintains performance improvement activities within the department.
Responsible for discharge planning of all patients to include review of social determinants of health. Ability to efficiently conduct plan of care meetings and work closely with utilization review nurse, director of nursing or their designee(s), and healthcare providers.

Education/Experience:
Bachelor's Degree or above
Current State Clinical Social Work Licensure.
Three or more years of previous experience in all aspects of social services and discharge planning.
